Physical Therapist – Home Health

Position Overview

We are seeking a Physical Therapist for home health visits across a multi-area service territory. This is a 13-week contract starting October 12, 2026, with day shift hours (7:00 AM – 7:00 PM).

Service Area

You will provide care across two assigned territories:

• Area #1: Fremont, Castro Valley, Ashland, Cheyeryland, Hayward, Newark, San Leandro, San Lorenzo, Union City
• Area #2: Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont

Travel outside the assigned area may be required. Weekend coverage may be necessary to maintain full patient caseload expectations.

Compensation & Scheduling

Two options are available:

Guarantee Option: 36 guaranteed hours per week; minimum 27 productivity points required. You must meet this threshold weekly or complete additional visits/weekend work as needed. If you decline patients, the guarantee does not apply. (Guarantee excludes the first and last week of assignment, holiday weeks, and weeks with approved time off.)

Non-Guarantee Option: Per-visit payment with flexible scheduling. You may refuse patients but must be willing to see COVID+ and COVID-PUI patients to build a full caseload.

Visit types convert to points and approved hours (e.g., routine evaluations and recertifications = 1 point/1.33 hours; OASIS SoC = 1.5 points/2 hours). All rates are all-inclusive and cover drive time, visit time, documentation, and patient communication.

Required Credentials

• Current California PT License
• BLS Certification
• Valid Driver's License and active Auto Insurance & Registration
• Annual TB test, MMR, Varicella, HepB, TDap (10 years), Annual Flu vaccine
• Drug screen and physical

All requested time off must be disclosed at time of submission. Late time-off requests will not be approved.
