Physical Therapist – Home Health

Position Overview

The facility is seeking a Physical Therapist for home health services covering the greater East Bay area, including Fremont, Castro Valley, Hayward, Newark, San Leandro, Union City, Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont. You will provide skilled physical therapy services to patients in their homes, managing evaluations, treatment, and discharge planning.

Schedule & Assignment

• Shift: Day (hours as arranged by caseload)
• Contract Duration: 91 weeks
• Start Date: October 12, 2026
• Travel: You may be required to travel outside the assigned service area and cover weekend visits as needed to maintain caseload expectations.

Compensation & Productivity

Two options available:

Guarantee Track: 36 hours/week guaranteed, with a requirement to complete at least 27 points per week. Points are assigned by visit type (routine visits and follow-ups = 1 point/1.33 hours; OASIS Start of Care = 1.5 points/2 hours). You must be willing to travel outside the service area or work weekends to meet weekly expectations.

Non-Guarantee Track: Per-visit payment with flexibility to refuse patients, though you may need to travel occasionally to maintain a full caseload.

All rates are all-inclusive and cover drive time, visit time, documentation, and patient communication.

Requirements

• Current California PT License
• BLS Certification
• Valid Driver's License, auto insurance, and vehicle registration
• Annual TB test, MMR, Varicella, HepB, TDap (within 10 years), annual flu shot
• Drug screen and physical exam
• Willingness to see COVID-positive and COVID-suspected patients
