Physical Therapist – Home Health

Position Overview

The facility is seeking a Physical Therapist for home health visits across a multi-county service area. This is a day shift role providing direct patient care in patients' homes.

Assignment Details

• Duration: 13 weeks
• Start Date: October 12, 2026
• Shift: Day (7:00 AM – 7:00 PM)
• Service Area: Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont, Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, and Kensington. Additional travel outside the assigned area may be required.
• Weekend Coverage: May be required

Productivity & Compensation

• Guaranteed 36 hours per week if you complete at least 27 points/week
• Paid on a per-visit basis; rate includes drive time, visit time, documentation, and phone calls
• Visit types convert to billable hours (OASIS visits typically = 1.33 hours; OASIS Start of Care = 2 hours)
• You may be required to travel outside the service area to maintain a full caseload
• Non-guarantee option available if preferred

Clinical Scope

Provide skilled physical therapy evaluations, re-evaluations, routine visits, and discharge planning. You will see patients across the continuum of care, including those who are COVID-positive or under investigation.

Required Credentials

• Current California PT License
• BLS Certification
• Valid Driver's License, auto insurance, and vehicle registration
• TB test (annual), MMR, Varicella, HepB, TDap (10-year), Flu vaccine (annual)
• Drug screen and physical exam

Important: All requested time off must be disclosed at time of submission. Contract dates must align exactly with agency agreement.
