Physical Therapist – Home Health

Position Overview

The facility is seeking a Physical Therapist for home health services in the greater Bay Area. You'll deliver skilled therapy to patients in their homes, managing evaluations, re-certifications, routine visits, and discharges across an assigned territory.

Assignment Details

• Service Areas: Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, and Kensington; plus either Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont OR Vallejo, Benicia, Napa, American Canyon, and Yountville
• Shift: Day (7:00 AM – 7:00 PM)
• Contract: 13 weeks
• Start: October 12, 2026
• Travel: Required within assigned service area; occasional travel outside area may be needed
• Weekends: Coverage may be required

Productivity & Compensation

• Minimum 27 points per week (36-hour guarantee for compliant travelers)
• Payment structure covers all time: travel, patient visits, documentation, and care coordination
• Point conversion: Standard visits = 1 point (1.33 hours); OASIS Start of Care = 1.5 points (2 hours)

Clinical Scope

You'll perform OASIS assessments, skilled evaluations, re-certifications, routine follow-ups, and discharge services. You must be willing to treat COVID-positive and COVID-PUI patients.

Required Credentials

• Current California PT License
• BLS Certification
• Valid driver's license, auto insurance, and vehicle registration
• Annual TB test, MMR, Varicella, Hepatitis B, Tdap (10-year), and annual Flu vaccine
• Drug screen and physical exam
