Physical Therapist – Home Health

Position Overview

We're seeking a Physical Therapist for home health visits across multiple service areas in the Bay Area. You'll provide direct patient care in patients' homes, managing a caseload of evaluation, treatment, and discharge visits.

Assignment Details

• Service Territory: Area #8 (Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington) plus either Area #2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont) or Area #7 (Vallejo, Benicia, Napa, American Canyon, Yountville)
• Schedule: Day shift; 36-hour weekly guarantee for compliant travelers
• Travel: May be required outside assigned areas to maintain full caseload
• Weekend Coverage: May be required
• Contract Duration: 13 weeks
• Start Date: October 12, 2026

Productivity Expectations

• Minimum 27 points per week (points based on visit type)
• OASIS assessments, evals, re-evals, and discharge visits
• Payment includes drive time, visit time, documentation, and patient/physician communication
• Pay is per-visit; must be willing to see COVID-positive and suspected COVID patients

Required Credentials

• Current California PT License
• BLS Certification
• Valid Driver's License and auto insurance
• Annual TB test, MMR, Varicella, HepB, TDap (10-year), annual flu shot
• Drug screen and physical exam
• Clean driving record
