Licensed Vocational Nurse (LVN) — Skilled Nursing Facility

Position Overview

The facility is seeking a compassionate and skilled Licensed Vocational Nurse to provide individualized nursing care to skilled nursing facility residents. You'll work closely with physicians, nursing staff, residents, families, and interdepartmental teams to deliver high-quality patient care under the direction of the Charge RN and Director of Nursing.

Key Responsibilities

  • Conduct individualized patient assessments, prioritizing data collection based on immediate needs and notifying leadership of abnormal findings
  • Develop and implement personalized care plans in collaboration with the healthcare team
  • Perform nursing procedures including EKGs, blood draws, IV starts, and wound care
  • Administer medications accurately and maintain proper documentation
  • Respond promptly to patient calls and needs; maintain clean, organized work areas
  • Provide patient and family education in a non-judgmental, culturally sensitive manner
  • Maintain strict confidentiality and adhere to safety and infection control standards
  • Participate in emergency response and resuscitation when needed
  • Assist with patient transfers for procedures and diagnostic tests
  • Contribute to staff education and orientation of new personnel
  • Attend shift rounds at 0630 and 1830 to ensure continuity of care

Required Qualifications

  • Current California LVN license
  • Valid BLS-level CPR certification
  • Ability to walk, bend, lift, and perform manual patient care skills
  • Strong communication skills in English, both verbal and written
  • Excellent assessment and problem-solving abilities
  • Reliable attendance and dependable work ethic
  • Ability to work effectively with diverse patient populations, including those with confusion or challenging behaviors
  • Commitment to continuing education and professional development

Position Details

  • Shift: 5 × 8-hour days
  • Contract Duration: 13 weeks
  • Start Date: October 1, 2026
  • Guaranteed Hours: 36 hours per week
  • Dress Code: Caribbean Blue scrubs (required)
  • Schedule: Flexibility required to meet department needs; willingness to work additional hours and support other areas when necessary
