Medical Social Worker – Case Management
Position Overview
The facility is seeking a Medical Social Worker to join its case management team on a 13-week travel assignment. You'll work day shifts (8:00 AM – 4:30 PM), Monday through Friday, with every other weekend required. Holiday coverage commitment is mandatory (3 of 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).
Key Responsibilities
- Conduct biopsychosocial assessments and develop individualized care plans
- Manage a caseload of up to 25 patients across multiple units (may float between up to 2 units on weekends)
- Lead discharge planning and contingency planning activities
- Facilitate family meetings and participate in interdisciplinary treatment planning
- Coordinate services and resources (home care, equipment, transportation, DCF involvement)
- Document care delivery and ensure compliance with evidence-based coverage criteria
- Educate patients and caregivers on benefits eligibility, prior authorizations, and community resources
- Work collaboratively with nursing, therapy, pharmacy, and social services teams
Required Qualifications
- Master of Social Work (MSW) and valid California state license (or compact license at submission)
- Minimum 2 years acute hospital case management experience
- Proficiency with computerized documentation systems
- Knowledge of 5150/involuntary hold procedures
- Experience with diverse patient populations: adults, pediatrics, geriatrics, neonates
- Ability to manage patients with substance abuse history, homelessness, and mental health crises
- Strong organizational and communication skills

- Contract: 13 weeks

- Scrubs or business casual attire
- RTO requests must be submitted upfront; maximum 7 days
Pending licenses accepted. California state license preferred.
