Case Manager – Acute Care
Position Overview
The facility is seeking an experienced Case Manager to join the care coordination team on a 13-week contract. You'll work day shifts (8:00 AM – 4:30 PM), Monday through Friday, with every-other-weekend coverage and holiday availability required.
Key Responsibilities
- Manage a caseload of up to 25 patients across multiple units
- Develop and execute discharge planning with focus on safe transitions to home, long-term care, or rehabilitation services
- Conduct comprehensive biopsychosocial assessments and participate in treatment plan development
- Facilitate family meetings and coordinate care with internal and external team members
- Assess patient vulnerability, safety risks, and social determinants of health
- Assist patients with benefits navigation, prior authorizations, and access to community resources
- Support patients experiencing homelessness, substance use disorders, or mental health crises
- Document thoroughly and ensure all care coordination activities align with evidence-based guidelines
Required Qualifications
- 2+ years of acute hospital case management experience
- Master of Social Work (MSW) or relevant case management credential
- California state license or pending license acceptable
- Experience with 5150/involuntary holds, admission criteria, and discharge planning
- Proficiency with electronic health record documentation
- Strong organizational and communication skills
Schedule & Expectations
- Day shift: Monday–Friday, 8:00 AM – 4:30 PM
- Every other weekend (focus on discharge planning)
- Must commit to working 3 of 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- May float between nearby facilities as needed
- RTO requests must be submitted upfront during application; post-offer changes may not be granted
What You'll Work With
Interdisciplinary teams including social services, physical therapy, respiratory, pharmacy, and rapid response. Patient populations span neonates through geriatrics.
