Care Manager RN
Position Overview
The facility is seeking a Care Manager RN to join its clinical team. You'll work collaboratively with interdisciplinary healthcare team members to facilitate patient care through effective utilization and monitoring of resources. In this role, you'll assume a leadership position in discharge planning and coordinate care across the acute care episode and continuum of services.
Key Responsibilities
- Collaborate with internal and external healthcare team members to optimize patient outcomes
- Monitor and coordinate healthcare resources during patient stays
- Develop and execute safe discharge plans aligned with clinical and financial goals
- Float to other units and hospitals as needed
Requirements
- Associate degree in Nursing (BSN or related bachelor's degree in healthcare preferred)
- Registered Nurse license in Alabama or compact license (no temporary licenses)
- Minimum 2 years nursing experience
- AHA BLS or Red Cross CPR certification
- Experience in utilization review and/or case management preferred
Position Details
- Shift: 7:00 AM – 3:00 PM
- Schedule: 36 guaranteed hours per week; will consider 36 or 48-hour arrangements
- Contract Duration: 13 weeks
- Dress Code: Solid navy scrubs
- Patient-Facing: Yes
- Float Requirement: Must be willing to float to other units and hospitals
- Parking: Available
