Physical Therapist – Long-Term Care & Rehabilitation
Position Overview
The facility seeks a Physical Therapist to join its rehabilitation team providing direct patient care in a long-term care setting. You'll screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs, developing individualized plans of care grounded in evidence-based practice and regulatory standards.
Clinical Responsibilities
- Perform comprehensive patient evaluations using standardized assessments
- Administer rehabilitation procedures including manual techniques, therapeutic exercises, ambulation training, modalities, and wound care
- Manage patients requiring supportive and assistive devices
- Monitor patient progress and adjust treatments accordingly
- Evaluate treatment outcomes and modify services based on clinical response
- Provide patient and family education and consultation
- Prepare discharge summaries and comprehensive documentation
- Supervise physical therapist assistants and rehabilitation aides as needed
- Participate in patient care conferences, utilization reviews, and rehabilitation meetings
Requirements
- Valid Massachusetts state physical therapy license
- Minimum 1 year of clinical PT experience
- Experience in long-term care or rehabilitation settings
- New graduates welcome if Level II clinical experience completed (not Level I observation only)
- Adherence to APTA practice standards and state regulations
Position Details
- Schedule: Monday–Friday, 7:00 AM – 3:00 PM (8-hour days)
- Contract: 13 weeks
- Start Date: October 5, 2026
- Location: Haverhill, MA
