Physical Therapist – Long-Term Care / Rehabilitation
Position Overview
The facility is seeking a Physical Therapist to join its rehabilitation team. You'll screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs, developing individualized plans of care grounded in evidence-based practice and regulatory standards.
Clinical Scope
Your responsibilities will include:
- Performing manual techniques, therapeutic exercises, and modalities
- Assisting patients with ambulation and functional mobility
- Providing wound care and patient education on supportive and assistive devices
- Assessing progress, adjusting treatments, and evaluating outcomes
- Documenting clinical information and maintaining accurate patient records
- Consulting with patients, families, and the interdisciplinary care team
- Preparing discharge summaries
You'll also supervise physical therapist assistants and rehabilitation aides as needed, and participate in patient care conferences, utilization reviews, and facility training.
Requirements
- Massachusetts state license (required)
- Minimum 1 year of PT experience
- New graduates welcome if you have completed Level II clinical experience
- Long-term care or rehabilitation experience (required)
- APTA membership and adherence to practice standards and code of conduct

- Schedule: Days, 7:00 AM – 3:00 PM, 8-hour shifts
- Duration: 13 weeks
- Start: October 5, 2026
- Location: Haverhill, MA
- On-call/Weekend: None required
