Position Overview
The facility is seeking a Physical Therapist to evaluate, treat, and rehabilitate patients with physical functioning disorders and wound care needs in a long-term care or rehabilitation setting.
Clinical Scope
You will screen and evaluate patients to develop comprehensive plans of care aligned with regulatory and clinical standards. Using standardized assessments and evidence-based practice, you'll administer a range of interventions including manual techniques, ambulation training, therapeutic exercises, modalities, assistive device fitting, and wound care. You'll assess patient progress, adjust treatments as needed, and evaluate outcomes to modify services accordingly.
Key Responsibilities
- Develop and execute individualized rehabilitation treatment plans
- Document all clinical information and treatment minutes in the electronic system per regulatory requirements
- Provide counseling and consultation to patients, families, caregivers, and interdisciplinary team members
- Participate in patient care conferences, utilization review meetings, and rehabilitation team meetings
- Supervise and support physical therapist assistants, rehabilitation aides, and students
- Maintain compliance with APTA and Oklahoma practice standards
Requirements
- Oklahoma State Physical Therapist license (required)
- Minimum 1 year of PT experience
- Long-term care or rehabilitation experience required
- New graduates welcome with Level II clinical experience
Shift & Schedule
- 7:00 AM – 3:00 PM, Monday–Friday
- 13-week contract
- Start date: October 5, 2026
