Acute Care Case Manager – 13-Week Contract
Position Overview
The facility is seeking an experienced Acute Care Case Manager for a day shift, 5 days per week (7:00 AM – 3:00 PM) position. You will manage a caseload of approximately 25 patients daily, coordinating care across the continuum and ensuring smooth transitions for patients of all ages—from neonates through geriatric populations.
Key Responsibilities
- Conduct patient assessments and develop individualized care plans in collaboration with interdisciplinary teams
- Manage discharge planning, utilization review, and prior authorizations
- Arrange DME, skilled nursing facility placement, and transport independently
- Organize and facilitate family meetings and treatment plan development
- Coordinate with Physical Therapy, Social Services, and clinical staff
- Navigate HMO/IPA frameworks, CMS guidelines, and DRG processes
- Demonstrate knowledge of adoption/foster care, substance abuse treatment options, homelessness resources, mental health crisis intervention, advance directives, and protective services
Required Qualifications
- Minimum 2 years of acute care hospital case management/discharge planning experience
- Hospital-based experience required (behavioral health experience does not apply)
- California state license (pending license acceptable; must be licensed by start date)
- Seasoned travel healthcare professional (not a first-time traveler)
- Two professional references covering the past year
Schedule & Expectations
- Day shift: 7:00 AM – 3:00 PM, Monday–Friday
- Every-other-weekend rotation to meet departmental needs
- 13-week contract; start October 5, 2026
- Restricted time off: maximum 7 days total
- Must commit to working 3 of 5 holidays (Thanksgiving, Christmas Eve/Day, New Year's Eve/Day)
Work Environment
Business attire or scrubs. Computerized documentation required. You may be asked to float to nearby facilities.
