Occupational Therapist – Long-Term Care/Rehabilitation

Position Overview

The facility is seeking an Occupational Therapist to join its rehabilitation team. You'll work days, 8-hour shifts, 35 hours per week, for a 13-week contract beginning October 5, 2026.

Clinical Responsibilities

You will screen and evaluate patients with cognitive and/or activities of daily living (ADL) and physical functioning disorders, developing individualized plans of care based on standardized assessments and evidence-based practice.

Clinical interventions include:

  • ADL retraining and therapeutic activities
  • Therapeutic exercises and modalities
  • Assessment and training with supportive and assistive devices
  • Patient and family counseling regarding physical disorders and adaptive strategies

You'll assess patient progress regularly, adjust treatments accordingly, and evaluate outcomes to guide service modifications and referrals. Comprehensive discharge summaries will be completed per regulatory standards.

Team & Documentation

Collaborate with supervisors and the interdisciplinary team during patient care conferences, utilization reviews, and rehabilitation meetings. Maintain accurate documentation in the electronic system, including treatment minutes, in full compliance with regulatory and clinical requirements.

You may supervise occupational therapy assistants, rehabilitation aides, and students as needed.

Requirements

  • Active Oklahoma OT license (required at submission)
  • 1+ years of OT experience
  • Long-term care or rehabilitation experience required
  • New graduates considered if they have completed a Level II fieldwork experience
  • NBCOT and state practice act adherence

Schedule: 7:00 AM – 3:00 PM, Monday–Friday; no on-call or weekend requirements.
