Location: Grand Junction, CO
Schedule: Monday–Friday, 7:00 AM–3:00 PM, with occasional weekend and on-call coverage
Contract Duration: 12 weeks
Guaranteed Hours: 40 per week
About the Role
The facility is seeking a compassionate Hospice Nurse to serve as a Primary RN, managing an assigned caseload of patients and leading the interdisciplinary care team. You'll provide skilled nursing care across multiple settings—private homes, nursing homes, group homes, and assisted living facilities—focusing on symptom management, comfort, and quality of life.
Key Responsibilities
- Assess and manage complex symptoms across all disease processes
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Educate patients and families about goals of care and empower caregivers with confidence
- Demonstrate strong clinical assessment and critical thinking skills
- Build meaningful relationships that honor patients' values and wishes
