Acute Care Case Manager — Travel Contract

Torrance, CA | Days | 13-Week Contract

This acute care facility in Torrance, CA is seeking an experienced Case Manager for a day-shift travel assignment beginning ASAP. The position operates Monday through Friday, 8:00 AM – 4:30 PM, with every-other-weekend coverage required. Candidates must also be available for holiday shifts.

• Weekend Requirement: Every other weekend; holiday availability required
• Float: May be required to float between nearby sister facilities; refusal may affect guaranteed hours

Clinical Scope

Case Managers carry a caseload of up to 25 patients across a broad range of age groups — neonates through geriatrics. Core responsibilities include:

• Admission review and medical necessity determination using evidence-based criteria
• Care coordination, discharge planning, and treatment plan development
• Arranging DME, SNF placement, and ambulance transport independently
• Familiarity with HMOs, IPAs, Medicare, and Medi-Cal coverage
• Supporting patients with behavioral health histories, homelessness, substance use, advance directives, and protective services needs
• Participating in family meetings and interdisciplinary rounds
• Weekend coverage may include monitoring multiple units with a focus on discharge planning

Requirements

• Minimum 2 years of acute care case management/discharge planning experience
• InterQual criteria proficiency strongly preferred
• Active California RN or Case Management license required at start (pending accepted at submission)
• Experience with admissions from ED, Mother Baby, and NICU settings preferred
• Acute behavioral health experience alone does not satisfy acute hospital requirements
• First-time travelers considered with screening
