Case Management – Discharge Planner | Atlanta, GA

Day Shift | Monday–Friday | 11-Week Contract

This Case Management opportunity focuses on discharge planning and care coordination for an adult and geriatric patient population. The role is Monday through Friday, days (8-hour shifts), with no weekend or call requirements.

Clinical Scope

  • Assess, evaluate, and drive individualized discharge plans from admission through transition
  • Coordinate services to match each patient's appropriate level of care
  • Initiate, evaluate, and re-evaluate treatment plans on a daily basis
  • Monitor length of stay, identify barriers to discharge, and implement solutions
  • Apply Medicare regulatory guidelines including patient choice, IMM notices, and MOONS
  • Conduct concurrent and retrospective reviews utilizing InterQual and Milliman criteria
  • Support appeals and denials, DRG and ICD-10 coding knowledge, and medical necessity determinations

Schedule & Float

  • Monday–Friday, 8-hour days (08:00–16:30)
  • Float within the facility as needed within scope of practice
  • No weekend shifts; no call

Requirements

  • Minimum 3 years of case management/discharge planning experience; first-time travelers not accepted
  • Active Georgia RN license or Compact license required
  • BLS certification required
  • Travel experience required

Additional Details

  • RTO must be finalized at time of submission; maximum 7 days per contract; no RTO in September
  • Holiday coverage expectations apply based on number of major holidays falling within the assignment period
  • Full compliance documentation required prior to start
