Case Management – Discharge Planner | Atlanta, GA
Day Shift | Monday–Friday | 11-Week Contract
This Case Management opportunity focuses on discharge planning and care coordination for an adult and geriatric patient population. The role is Monday through Friday, days (8-hour shifts), with no weekend or call requirements.
Clinical Scope
- Assess, evaluate, and drive individualized discharge plans from admission through transition
- Coordinate services to match each patient's appropriate level of care
- Initiate, evaluate, and re-evaluate treatment plans on a daily basis
- Monitor length of stay, identify barriers to discharge, and implement solutions
- Apply Medicare regulatory guidelines including patient choice, IMM notices, and MOONS
- Conduct concurrent and retrospective reviews utilizing InterQual and Milliman criteria
- Support appeals and denials, DRG and ICD-10 coding knowledge, and medical necessity determinations
Schedule & Float
- Monday–Friday, 8-hour days (08:00–16:30)
- Float within the facility as needed within scope of practice
- No weekend shifts; no call
Requirements
- Minimum 3 years of case management/discharge planning experience; first-time travelers not accepted
- Active Georgia RN license or Compact license required
- BLS certification required
- Travel experience required
Additional Details
- RTO must be finalized at time of submission; maximum 7 days per contract; no RTO in September
- Holiday coverage expectations apply based on number of major holidays falling within the assignment period
- Full compliance documentation required prior to start
