Position Overview
Join the facility's case management team in Asheville, NC. This 13-week contract offers a day shift (7:00 AM – 3:00 PM) supporting care coordination and patient transitions across the health system.
What You'll Do
Work collaboratively with nurses, physicians, and interdisciplinary teams to facilitate safe, efficient patient care. You'll manage documentation, communicate with patients and families, and coordinate resources to support positive patient outcomes. Your ability to prioritize multiple tasks, build trust with patients and team members, and maintain strong communication will be essential.
