Position Overview
The facility is seeking a Case Manager for a 13-week assignment. This is a day shift role (7:00 AM – 3:00 PM) offering 36 guaranteed hours per week with self-scheduling flexibility.
About the Role
You'll work collaboratively with the clinical and medical teams to support patient care coordination and outcomes. Your day-to-day responsibilities will include:
- Strong documentation and communication skills
- Managing multiple priorities and shifting focus between tasks
- Building trust with patients, families, and the care team
- Supporting the hospital's commitment to high-quality, safe patient care
What We're Looking For
- Ability to work collaboratively across disciplines (RN, MD, case management teams)
- Excellent organizational and time-management skills
- Professional communication with patients, families, and staff
- No specific certifications required
- Business attire required
