Case Management | Eureka, CA | Day Shift | 13-Week Contract
This acute care Case Management role is based in Eureka, California — a stunning coastal city surrounded by redwood forests, national parks, and rugged Pacific coastline.
Shift & Schedule
- Days, 5x8-hour shifts (08:00–16:30)
- Every other weekend required
- No on-call required
- Must commit to 3 of 5 winter holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- All RTO must be submitted at time of application
Patient Population & Caseload
- Caseload of up to 25 patients per day
- Ages served: pediatrics, adolescents, adults, and geriatrics
- Interdisciplinary support includes Social Services
Required Experience & Skills
- Minimum 2 years of acute care hospital Case Management experience; experience must be current within the last year
- Experience with HMOs, IPAs, advance directives, POLST, and protective services processes
- Proficiency in discharge planning, care coordination, appeals and denials, prior authorizations, DRG, and CMS guidelines
- Floating between units or nearby sister facilities may be required
Certifications & Licensing
- Valid compact RN license required and must be in hand at time of submission
- 2 professional references covering at least 1 year within the last 3 years required prior to start
