Case Manager – Acute Care | Eureka, CA | Day Shift | 13-Week Contract

This is a day-shift Case Management opportunity in Eureka, California, ideal for an experienced acute care case manager ready to take on a meaningful clinical role in a scenic Northern California setting.

Shift & Schedule

• Days, 5x8-hour shifts (08:00–16:30)
• Every other weekend required
• No on-call required
• Holiday commitment required (3 of 5: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)

Patient Population & Caseload

• Adolescents, adults, pediatrics, and geriatrics
• Caseload of up to 25 patients
• Interdisciplinary collaboration with social services

Key Responsibilities

• Acute care case management including care coordination and discharge planning
• Appeals and denials, prior authorizations, and benefits eligibility
• Compliance with CMS, HIPAA, and Joint Commission guidelines
• Experience with HMOs, IPAs, advance directives, POLST, and protective services processes
• DRG knowledge and worker's compensation case management preferred

Requirements

• Minimum 2 years of acute care hospital case management experience, current within the last year — this is a hard requirement
• Valid compact RN or applicable license in hand at time of submission
• Travel experience required
• Two professional references covering at least 1 year within the last 3 years
• Locals within 50 miles accepted

Floating

Travelers may float to sister facilities nearby based on departmental need. Refusal to float may impact guaranteed hours.

All RTO must be submitted at time of application. Post-offer RTO requests may not be accommodated.
