Care Manager – Acute Medical Case Management

Position Overview

The facility is seeking an experienced Care Manager to join its acute medical care coordination team. You'll manage a caseload of up to 25 adult and geriatric patients, providing discharge planning, care coordination, and medical necessity assessments in a fast-paced acute care environment.

Shift & Schedule

  • Days: Monday–Friday, 7:00 AM – 3:00 PM (5×8-hour shifts)
  • Weekend Coverage: Every other weekend (Saturday and Sunday), with a weekday day off in exchange
  • Contract Duration: 26 weeks
  • Start Date: ASAP

Requirements

  • California state license (pending licenses accepted; must be active upon start)
  • Minimum 2 years acute medical Care Manager experience in care coordination and discharge planning
  • Strong knowledge of HMO/IPA models, CMS guidelines, and DRG coding
  • Proficiency with computerized documentation systems
  • Two professional references (covering at least 1 year within the last 3 years)
  • Willingness to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)

Key Responsibilities

  • Conduct patient assessments and develop individualized plans of care
  • Coordinate discharges and arrange DME/post-acute services
  • Apply evidence-based guidelines to determine medical necessity
  • Maintain accurate, timely documentation
  • Collaborate with interdisciplinary care teams
