Care Manager – Acute Medical Case Management
Position Overview
The facility is seeking an experienced Care Manager to join its acute medical care coordination team. You'll manage a caseload of up to 25 adult and geriatric patients, providing discharge planning, care coordination, and medical necessity assessments in a fast-paced acute care environment.
Shift & Schedule
- Days: Monday–Friday, 7:00 AM – 3:00 PM (5×8-hour shifts)
- Weekend Coverage: Every other weekend (Saturday and Sunday), with a weekday day off in exchange
- Contract Duration: 26 weeks
- Start Date: ASAP
Requirements
- California state license (pending licenses accepted; must be active upon start)
- Minimum 2 years acute medical Care Manager experience in care coordination and discharge planning
- Strong knowledge of HMO/IPA models, CMS guidelines, and DRG coding
- Proficiency with computerized documentation systems
- Two professional references (covering at least 1 year within the last 3 years)
- Willingness to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
Key Responsibilities
- Conduct patient assessments and develop individualized plans of care
- Coordinate discharges and arrange DME/post-acute services
- Apply evidence-based guidelines to determine medical necessity
- Maintain accurate, timely documentation
- Collaborate with interdisciplinary care teams
