Care Manager – Acute Care

Position Overview

The facility is seeking an experienced Care Manager to join its acute care team on a day shift basis. You will manage a caseload of up to 25 adult and geriatric patients, performing care coordination, discharge planning, and utilization management in a fast-paced hospital setting.

Schedule & Duration

• Shift: Monday–Friday, 8:00 AM–4:30 PM (5x8-hour days)
• Contract Length: 13 weeks
• Start Date: ASAP
• Weekend Requirement: Every other Saturday and Sunday, with a weekday off in exchange

Requirements

• Current California state license (or pending license acceptable if licensed by start date)
• Minimum 2 years acute care experience in Care Coordination and Discharge Planning
• Demonstrated experience with care coordination, admission criteria, discharge planning, and disease management
• Knowledge of HMOs, IPAs, and CMS/DRG guidelines
• Proficiency with electronic health records (computerized documentation required)
• Travel experience required
• Valid references (2 required, covering at least 1 year within the last 3 years)
• Ability to commit to working at least 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)

What to Expect

Day shift, no float requirement. You'll assess patients, coordinate care transitions, and manage referrals for a diverse adult and geriatric population. Business attire or scrubs; facility will provide communication devices as needed.
