Speech Language Pathologist – Long-Term Care

Position Overview

The facility seeks a Speech Language Pathologist for its long-term care setting. This is an 8-hour day shift, Monday through Friday, 7:00 AM – 3:30 PM. Contract duration is 13 weeks.

Key Responsibilities

  • Conduct comprehensive speech and language assessments for long-term care residents
  • Develop and implement individualized treatment plans addressing communication and swallowing disorders
  • Collaborate with nursing, medical, and house staff to integrate therapy goals into resident care plans
  • Document patient progress and maintain accurate clinical records
  • Apply knowledge of the PDPM reimbursement system in treatment planning and documentation

Required Qualifications

  • Bachelor's degree in Speech Language Pathology from a college/university accredited by the American Speech-Language-Hearing Association (ASHA)
  • Current New York State license as a Speech-Language Pathologist
  • Certificate of Clinical Competence in Speech Language Pathology from ASHA (or eligibility)
  • Long-term care experience required
  • Proficiency with MS Office and EMR software
  • Excellent assessment, communication, and clinical reasoning skills
