Speech Language Pathologist – Long-Term Care
Position Overview
The facility seeks a Speech Language Pathologist for its long-term care setting. This is an 8-hour day shift, Monday through Friday, 7:00 AM – 3:30 PM. Contract duration is 13 weeks.
Key Responsibilities
- Conduct comprehensive speech and language assessments for long-term care residents
- Develop and implement individualized treatment plans addressing communication and swallowing disorders
- Collaborate with nursing, medical, and house staff to integrate therapy goals into resident care plans
- Document patient progress and maintain accurate clinical records
- Apply knowledge of the PDPM reimbursement system in treatment planning and documentation
Required Qualifications
- Bachelor's degree in Speech Language Pathology from a college/university accredited by the American Speech-Language-Hearing Association (ASHA)
- Current New York State license as a Speech-Language Pathologist
- Certificate of Clinical Competence in Speech Language Pathology from ASHA (or eligibility)
- Long-term care experience required
- Proficiency with MS Office and EMR software
- Excellent assessment, communication, and clinical reasoning skills
