Physical Therapist Assistant – Post-Acute Care

Position Overview

Join a skilled nursing facility in Sacramento, CA, treating post-acute patients in a collaborative, interdisciplinary setting. You'll work primarily in skilled nursing with the opportunity to float to an adjacent inpatient acute care hospital on the same campus.

Shift & Schedule

  • 8-hour days, 7:00 AM – 3:00 PM
  • Two weekend shifts per month
  • First-to-float role
  • 40 guaranteed hours per week
  • 13-week contract

Patient Population & Clinical Scope

You'll treat 8–9 patients daily with diagnoses including:

  • Total joint replacements (TKA, THA)
  • Stroke and CVA
  • COPD and ia
  • Deconditioning and balance disorders
  • General medical conditions in geriatric populations

Work focuses on functional mobility, transfers, and therapeutic exercise. You'll perform floor-specific orientation and work independently while collaborating closely with the rehab team.

Requirements

  • Current California PT license
  • AHA BLS certification
  • Ability to perform transfers and work independently
  • Strong communication and teamwork skills
  • Minimum 6 months experience in acute care or skilled nursing (new grads considered with appropriate background)
  • Two supervisor references from your modality (one within 12 months, one within 3 years)

  • Hospital may cancel up to 4 shifts per 13-week assignment
