Physical Therapist – Home Health
Position Overview
Join the home health team providing skilled physical therapy services to patients in their homes. You'll manage a dynamic caseload, conduct comprehensive assessments, develop individualized treatment plans, and deliver evidence-based interventions while coordinating care across the clinical team.
Shift & Schedule
- Monday–Friday, 8:00 AM–4:00 PM
- 2–3 weekend days required
- 13-week contract
- 37 guaranteed hours per week
Key Responsibilities
- Perform comprehensive OASIS and PT assessments, including psychosocial, safety, and functional evaluations
- Develop and implement individualized plans of care focused on functional independence and safety
- Provide skilled physical therapy treatment, including therapeutic techniques and patient/caregiver education
- Supervise PTAs, PT students, and home care aides
- Coordinate care with physicians, nurses, caregivers, and other rehabilitation services
- Manage patient visit schedules to optimize productivity and meet clinical needs
- Complete accurate OASIS documentation, clinical notes, and required forms in compliance with federal and state regulations
- Perform patient health assessments and procedures (vital signs, wound care, suture removal, etc.)
- Participate in quality improvement initiatives and peer review audits
- Engage in community outreach and in-service training activities
Required Qualifications
- Valid Physical Therapist license
- APTA membership preferred
- Current CPR certification
- Ability to travel to patient homes and manage an independent caseload
- Strong organizational and communication skills
