Care Manager – Case Coordination
Position Overview
The facility is seeking an experienced Care Manager to join its acute care team. You'll work day shift (7:00 AM – 3:00 PM) in a patient-centered care coordination role, supporting adults and geriatric patients with complex medical and social needs.
Key Responsibilities
- Assess patients and develop individualized care coordination plans
- Manage discharge planning and arrange post-acute services (SNF, DME, ambulance transport)
- Organize and facilitate family meetings and treatment plan development
- Navigate appeals, denials, and CMS guidelines
- Support patients experiencing homelessness, mental health crises, substance use disorders, and those in foster/adoptive situations
- Maintain accurate, timely documentation in the electronic health record
- Work independently and collaboratively across the interdisciplinary team
Requirements
- California state license (must be active at start date; pending licenses accepted if activated before day one)
- Minimum 2 years acute care case management and care coordination experience
- Current travel healthcare experience (not your first assignment)
- Strong knowledge of HMOs, IPAs, discharge planning, and advance directives (POLST)
- Competency with computerized documentation systems
- Ability to work a variable 5-day workweek; weekend coverage required (specific days off not guaranteed but consecutive days off scheduled when possible)
Contract Details
- Duration: 26 weeks
- Start: ASAP (September 21, 2026)
- Shift: Day 5x8 (7:00 AM – 3:00 PM)
- Dress code: Turquoise or business casual
- Patient ratio: 1:25
- Float to nearby facilities as needed; commitment to floating is mandatory
- Maximum 7 days RTO; must commit to working 3 of 5 holiday shifts (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
