Day Shift | 13-Week Contract | ASAP Start
This acute care facility is seeking an experienced travel Care Manager to join its Case Management team. This is a fast-paced, patient-centered role focused on care coordination and discharge planning for an adult and geriatric population.
Shift & Schedule
- Days, 8-hour shifts (08:00–16:30)
- Variable 5-day workweek; department operates 7 days per week
- Days off may vary and are not guaranteed to be consecutive
- Holiday commitment required: must work 3 of 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
Clinical Scope
- Caseload of up to 25 patients
- Patient assessment, care coordination, and discharge planning
- Facilitate and participate in family meetings and interdisciplinary treatment planning
- Arrange DME, SNF placement, and ambulance transport — independently
- Work with HMOs, IPAs, and community resources
- Knowledge of advance directives, POLST, protective services, mental health resources, homelessness resources, and substance abuse treatment options
- Note: This role does not include Utilization Review
Requirements
- Minimum 2 years of acute medical Care Manager experience in Care Coordination
- Prior travel experience required — first-time travelers not accepted
- Active California RN or CM license required by start date (pending accepted)
- Scrub color: Turquoise or Business Casual
Floating
Travelers may be expected to float to a nearby sister facility. Refusal to float may result in termination of assignment.
RTO
Maximum 7 days; all requests must be submitted upfront at time of submittal.
