Position Overview
The facility is a skilled nursing facility seeking an experienced Physical Therapist to join its rehabilitation team. You'll assess and treat residents to help them regain physical function, improve mobility, and achieve greater independence. Working collaboratively with occupational therapists, nurses, and physicians, you'll design and implement individualized treatment plans that enhance quality of life and support recovery.
Key Responsibilities
- Conduct thorough physical evaluations and develop personalized treatment plans
- Implement therapeutic interventions to improve strength, mobility, and functional abilities
- Educate patients and families on exercises, safe movement techniques, and home modifications
- Collaborate with the interdisciplinary care team to coordinate comprehensive resident care
- Monitor progress and adjust treatment plans to optimize outcomes
- Maintain accurate, timely documentation in compliance with facility and regulatory standards
Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Valid Physical Therapy license
- Experience in a skilled nursing facility or similar setting preferred
- Knowledge of PT techniques, therapeutic modalities, and adaptive equipment
- Strong communication, interpersonal, and problem-solving skills
- Ability to work independently and collaboratively
- Compassionate, patient-centered approach

- 13-week contract
