Case Management RN — Acute Care | Los Angeles, CA

Day Shift | 13-Week Contract

This position supports a fast-paced acute care case management department serving adolescent, adult, and geriatric patient populations. The role focuses on care coordination, discharge planning, and utilization management across a clinically diverse caseload.

Shift & Schedule

• Days, 5x8-hour shifts (08:00–17:00)
• No weekend, float, or on-call requirements
• No scheduling requests can be accommodated — flexibility is expected

Clinical Scope

• Comprehensive discharge planning and plan of care development
• Utilization review using InterQual and Milliman criteria
• Coordination with social services and transportation teams
• Familiarity with CMS guidelines, DRG and ICD-10 coding, CPT billing, and NCQA standards
• Adherence to HIPAA and Joint Commission/National Patient Safety Goals

Requirements

• Minimum 3 years of acute care case management experience
• Active, permanent California RN license (no pending or temporary licenses accepted)
• BLS certification
• Fluenza vaccine required — no exemptions
• Proficiency with electronic health records preferred

Submission Requirements

• CA RN license must be active and in hand at time of submission
• Two professional references from within the past year, at supervisor level or above, required at submission
• Any license or background discrepancies must be fully resolved prior to consideration

Time Off

• Maximum 2 weeks of requested time off per contract
• No RTO permitted during the first 2 weeks of the assignment
