Case Manager – Care Coordination | Carmichael, CA

Contract: 17 weeks | Shift: 8-hour Days, 7:00 AM – 3:00 PM | Start Date: September 15, 2026

This Case Management/Care Coordination role covers both Utilization Review and Discharge Planning for all inpatient areas. The caseload runs approximately 15–20 consultations per day, serving a diverse patient population that includes all payer types, uninsured individuals, and those experiencing homelessness.

Clinical Responsibilities:

• Complete initial assessments, Interqual-based utilization review, and discharge planning
• Coordinate care across all hospital departments (no outpatient or home care coverage)
• Collaborate with hospitalists, intensivists, and interdisciplinary teams
• Reprioritize fluidly throughout the day based on patient and unit needs

Schedule:

• Every other weekend required (minimum one full weekend per month)
• Holiday work not required, though voluntary participation is welcome
• First to float; hours fall within an 8:00 AM–5:00 PM window depending on department needs

Dress Code: Business casual attire

Orientation: Five days of care coordination new employee orientation; unit-specific orientation timeline is determined individually.

Requirements:

• BSN required
• Active California RN or applicable license
• BLS certification
• Familiarity with Interqual criteria (training provided for those needing a refresher)
• Prior recent experience at a California Dignity Health facility is required
• Strong communication, collaboration, and organizational skills
• 2 supervisory references required at submission (charge nurse accepted); one within 12 months, one within 3 years

Guaranteed Hours: 40 hours per week
