Physical Therapist – Skilled Nursing Facility

Location: Waltham, MA

Shift: Days (7:00 AM – 7:00 PM)

Contract Duration: 13 weeks

Start Date: August 31, 2026

About the Role

The facility is seeking a Physical Therapist to deliver direct care to residents in a skilled nursing setting. You'll evaluate residents, develop individualized therapeutic plans, and lead treatment programs focused on maximizing functional independence and activities of daily living.

Key Responsibilities

• Perform evaluations and establish therapeutic plans of care with functional goals aligned to resident needs
• Deliver therapeutic interventions under physician order, with attention to precautions and contraindications
• Collaborate with the interdisciplinary care team, attend team meetings, and consult with physicians on discharge planning
• Monitor treatment progress, modify plans as needed, and provide resident and caregiver education
• Document all evaluative and therapeutic interactions in the electronic medical record
• Recommend and instruct on assistive devices
• Supervise Physical Therapist Assistant staff and mentor new team members
• Support compliance with Medicare, Medicaid, and other reimbursement regulations
• Maintain infection control standards and equipment maintenance

Required Qualifications

• Bachelor's degree (Master's or Doctorate preferred)
• Active state PT license
• Prior experience in long-term care or skilled nursing facility (strongly preferred)
• Basic Life Support (BLS) certification preferred
• Strong communication, documentation, and interpersonal skills
• Proficiency with Microsoft Office

Physical Demands

Ability to sit, stand, walk, bend, stoop, twist, and reach. Lift up to 50 pounds frequently; lifts over 50 pounds require a second person. Some stress related to high responsibility for resident care.
