Position Overview
The facility is seeking a pharmacist for a 13-week contract assignment. This is a full-time position working 4 days per week, 10 hours per day (9:00 AM–7:00 PM).
Key Responsibilities
- Review, verify, and dispense medication orders for accuracy, appropriateness, and safety
- Provide clinical pharmacy services, including pharmacokinetic dosing recommendations and comprehensive drug interaction monitoring
- Monitor drug therapy regimens for contraindications, allergies, and appropriate dosing
- Counsel patients and families on medication use, side effects, and adherence
- Collaborate with physicians, nurses, and interdisciplinary teams on medication therapy management
- Manage anticoagulation services, medication reconciliation, and transitions of care
- Supervise pharmacy technicians and support personnel
- Participate in antimicrobial stewardship and quality improvement initiatives
- Detect and report adverse drug reactions per facility protocols
- Maintain accurate controlled substance records and inventory
- Ensure compliance with state and federal pharmacy regulations
Required Qualifications
- Doctor of Pharmacy (Pharm.D.) degree from an accredited program
- Texas state pharmacist license
- BLS certification (AHA); ACLS preferred
- 1+ years of hospital pharmacist experience
- Knowledge of pharmacokinetic dosing and neonatal medication management preferred
- Strong familiarity with pharmacy information systems and clinical decision support tools
