Physical Therapist – Home Health
Position Overview
We're seeking a licensed Physical Therapist to deliver skilled in-home physical therapy services to patients in the community. You'll work one-on-one with patients in their homes, providing evidence-based care that supports mobility, safety, and functional independence.
Clinical Responsibilities
- Perform evaluations, reassessments, recertifications, and discharge visits per physician orders
- Develop and implement individualized plans of care addressing patient-specific mobility and functional goals
- Deliver skilled interventions including gait training, balance training, therapeutic exercise, fall prevention, and mobility training
- Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
- Document all visits accurately and timely in the electronic health record system
- Communicate patient progress and any changes in condition to the interdisciplinary care team
- Collaborate with clinical staff (RNs, PTAs, OTs, STs) on care coordination and regulatory compliance
- Provide clinical oversight of PTAs as assigned and permitted under state law
Requirements
- Current, unrestricted Colorado Physical Therapist license
- Graduation from an accredited Physical Therapy program
- Valid driver's license and reliable transportation
- Ability to practice independently in a home-based setting
- Proficiency or willingness to learn the electronic documentation system
Preferred Qualifications
- Home health experience
- Familiarity with Medicare documentation and regulations
- Experience with geriatric and medically complex populations
Schedule & Contract
- Monday–Friday, 8:00 AM–5:00 PM
- 91-week contract
- Guaranteed 40 hours per week
