Occupational Therapist – Long-Term Care / Rehabilitation
Position Overview
The facility is seeking an Occupational Therapist to join its rehabilitation team. You'll evaluate and treat patients with cognitive and/or physical functioning disorders, developing and implementing individualized treatment plans to maximize independence and quality of life.
Shift & Schedule
- Days: 7:00 AM – 3:00 PM
- 8-hour shifts, 5 days per week
- 13-week contract
- No on-call or weekend requirements
Key Responsibilities
- Conduct comprehensive functional assessments using standardized, evidence-based tools
- Develop and adjust individualized treatment plans based on patient progress
- Provide therapeutic interventions including ADL retraining, therapeutic exercises, modalities, and adaptive equipment training
- Educate patients, families, and caregivers on adaptive techniques and home safety strategies
- Collaborate with physicians, physical therapists, and the interdisciplinary care team
- Document evaluations, progress notes, and discharge summaries in compliance with regulatory standards
- Participate in care conferences, utilization reviews, and facility in-services
- May supervise OT assistants, rehabilitation aides, and students
Required Qualifications
- Master's or Doctoral degree in Occupational Therapy from an ACOTE-accredited program
- NBCOT certification
- Active Oklahoma OT license
- BLS certification
- Minimum 1 year of OT experience (long-term care or rehabilitation preferred)
- Experience with electronic documentation systems
