Physical Therapist – Home Health

Position Overview

Join a home health team delivering skilled physical therapy services across the greater Silicon Valley area. You'll work directly with patients in their homes, evaluating and treating conditions to restore function, prevent disability, and help clients achieve their maximum independence following injury or illness.

About the Role

Your primary responsibilities include:

• Conducting initial evaluations and implementing medically prescribed physical therapy programs
• Documenting clinical observations, findings, treatment interventions, goals, and discharge planning
• Supervising Physical Therapy Assistants as needed
• Completing OASIS assessments and communicating with the clinical care team
• Providing skilled interventions tailored to each patient's capabilities and recovery goals

You'll work autonomously in patients' homes, managing your own caseload across a multi-city service territory including Sunnyvale, Palo Alto, Mountain View, Los Altos, Menlo Park, and surrounding communities.

Requirements

• Current Physical Therapist license (California)
• Minimum one year of home health experience
• Ability to travel between patient locations
• Valid driver's license and reliable transportation

Schedule & Contract

• Hours: 7:00 AM – 3:00 PM, days
• Weekend commitment: One weekend per contract period
• Schedule flexibility available based on patient needs
• Contract duration: 13 weeks
• Guaranteed hours: 40 per week
• Start date: September 8, 2026
