Case Management – Acute Inpatient | Jackson, MS
Position Overview
The facility seeks an experienced acute inpatient case manager to join a multidisciplinary team managing complex patient discharges and care coordination across all inpatient units. You'll work Monday–Friday, 7:00 AM–3:00 PM, with rotated holiday coverage as needed.
Key Responsibilities
- Manage a caseload of 15–20 patients per day across all inpatient units
- Coordinate discharge planning for patients with diverse medical diagnoses
- Consult with respiratory therapy, pharmacy, social work, and clinical teams as needed
- Facilitate placements to rehabilitation, long-term care, and hospice settings
- Leverage contracted community bed capacity and financial assistance programs for complex or underinsured patients
- Work closely with a 32-member social work team—you'll never work alone
Required Qualifications
- Active RN license
- Minimum 2 years acute inpatient medical case management experience
- At least 1 year (within the last 3 years) in a high-volume, complex acute-care setting
- Recent (within 2 years) discharge planning experience in acute care
- Previous travel assignment in a similar setting (within the last 3 years)
- BLS certification
- Proficiency with electronic health records and strong computer skills
- Ability to learn quickly and work independently within a supported team structure
Contract & Schedule
- 13 weeks | 40 guaranteed hours per week
- Monday–Friday, 7:00 AM–3:00 PM
- No weekend commitment
- 2-week unit-specific orientation
- Start date: August 26, 2026
Additional Requirements
- Flu vaccination (medical exemptions with documented evidence accepted)
- Fingerprinting (remote or in-person options available)
- 50-mile radius for travel assignments
